The Licensed Practical Nurses is directly responsible to the Director of Nursing.
Responsibilities of the Licensed Practical Nurse
Responsible and Accountable to:
Perform skilled nursing services under the supervision of a Registered Nurse including such duties as the following:
- Administer prescribed medications and treatments according to law and as permitted within the scope of the individual's license
- Perform nursing care according to the needs of the patient and as indicated in the written plan of care.
- Inform the physician and other personnel of changes in the patient's condition and needs
- Prepare clinical and/or progress notes and incorporate them into the clinical record at least weekly.
- Teach self-care techniques to the patient and/or family.
- Develop plans of care.
- Participate in in-service programs.
- Work under the supervision of a registered nurse.
- Observe, record, and report to the immediate supervisor the general physical or mental condition of the patients
- Assist the physician and/or registered nurse in performing specialized procedures.
- Assist in development of the plan of care.
- Prepare equipment for treatments, including sterilization and adherence to aseptic techniques.
- Assist the patient with activities of daily living and encourage appropriate self-care.
- Perform complex wound care if in-service is documented for specific procedure.
- May perform routine venipuncture (phlebotomy) if written documentation of competency is in personnel record. Competency must be evaluated by an R.N. seen if L.P.N. has completed a certification course.
Qualifications
- Current LPN license in the State where services will be provided
- One year clinical experience in home health preferred with at least one year clinical experience required
- Knowledge of home health federal, state, accreditation regulations
- Ability to effectively problem solve and possess decision making skills
- Demonstrates qualities of ethics and integrity, dependability and reliability
- Active communication skills
- Demonstrates excellent clinical and documentation skills
- Must agree to work at only one agency per day
The L.P.N. shall not:
- Access any intravenous appliance for any reason.
- Perform supervisory aide visit.
- Alter the plan of care, make initial assessment visit, prepare the recertification, make aide assignments, function as a supervisor of the nursing practice of any registered nurse, direct and supervise quality assurance programs, or act as a consultant to any home health agency.
