Description
To provide optimal nursing care to the terminally ill Hospice patient according to their individualized needs. To provide ongoing education and support to the patient and family. To work as a member of the Hospice team in providing Hospice care while upholding the company's standards of excellence, integrity, and compassion.
Essential Functions
- Must be able to exemplify compassion, patience, and sympathy to the patients and their families.
- Evaluate patients for hospice appropriateness.
- Perform assessments and provide hands-on care as directed by the physician.
- Be an advocate for patients and their families.
- Coordinates care with all IDG team members.
- Directly reports to the physician and primarily monitors the daily plan of care.
- Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet specific needs.
- Performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor's orders.
- Prepare accurate and timely clinical documentation and updates to the plan of care.
- Participates in on-call rotation.
- Participates in the quality assurance process.
Requirements
- Educational/Degree: Graduate from an accredited school of nursing.
- Training/Licensure: Currently licensed as a registered nurse to practice in the state with no restrictions.
- Knowledge/Skills/Ability: Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural and religious modes.
- Experience: Minimum 2 years of experience as a registered nurse, hospice experience preferred.
- Active patient contact within past two years, preferred.
