Description
The Clinical Manager is a qualified individual who provides oversight of all patient care services and personnel for their specific team. He/she serves as a leader for the team of caregivers. They will direct ongoing patient care, evaluates care provided, modifies the plan of care and evaluates the performance of each team member. The Clinical Manager must have at least one year of Home Health experience.
Job Relationships
- Supervised by: The Director of Nursing
- Workers Supervised: Registered Nurses, Licensed Practical Nurses and Certified Nursing Assistants on his/her team.
- Interrelationships: All members of the agency's care teams and management.
Responsibilities and Duties
- Make patient and personnel assignments.
- Coordinate all patient care for patients on his/her team.
- Coordinate referrals.
- Assure that patient needs are continually assessed.
- Assure the development, implementation, and updates of the individualized plan of care.
- Provide or supervise skilled nursing services in accordance with physician's orders.
- Consult with the physicians, field staff, social workers, discharge planners and other disciplines to establish a coordinated plan of care.
- Develop the patient's plan of care and recertification plans of care based upon patient needs evidenced upon evaluation and following consultation with the referring physician.
- Conduct ongoing evaluation and planning for the patients by daily conferences with the field staff and communication with the patient's physician.
- Continually assesses the goals and objectives of care for the patients and guides the field staff in discharge planning for the patients.
- Evaluates all paperwork completed by team members for completion and accuracy.
- Conducts all follow-up communications with the physicians unless additional input is needed from a field staff clinician.
- Prepares weekly visit schedules for visiting field staff. Monitors visits to ensure compliance to the plan of care in regards to visit frequency.
- Schedules LPN and Aide supervisory visits, ensuring compliance to rile and regulation.
- Conducts bi-weekly team conferences with the visiting staff caregivers.
- Attends bi-weekly team leader meetings.
- Records and reports patient and employee infections, incidents and accidents.
- Participates in staff meetings, in-services and continuing education offerings.
- Participates in quality assurance activities through data collection and participation in data analysis as opportunity is presented.
- Serves as a field visiting staff RN when the need arises to ensure the continuance of patient care.
Qualifications
- Must hold a current license issued by the Louisiana State Board of Registered Nurse Examiner's without restrictions
- Must have a minimum of one year's experience as a Registered Nurse. Must have at least one year of current experience in Home Health.
